Itemized vs bundle — ask which one you hold
Itemized means each step has its own line: exam, imaging, extraction, implant body, abutment, crown, temporary, lab fee, anesthesia, and so on. You can see what happens if one tooth changes.
Bundle / package language (“full mouth package,” “All-on-4 deal”) is fine only when the bundle still lists what is inside and what triggers extras. If the clinic will not unbundle on paper, you cannot compare clinics or dispute a mid-treatment surprise.

Brand and lab must appear on the paper
“Premium implant” and “high-quality zirconia” are marketing, not line items. Ask for:
- Implant brand and system (when implants are planned) — context: Straumann vs generic implants.
- Crown / bridge material (zirconia, PFM, etc.) named, not “ceramic smile.”
- Whether the lab is in-house or external, and whether the lab name can appear on the plan.
- Cement-retained vs screw-retained when that choice exists.
If brand fields are blank “until surgery day,” treat that as unfinished paperwork — not a reason to rush the deposit.
Temporary vs final prosthesis
Full-arch and many implant cases split into stages. Your plan should say, in plain English:
- What you leave with today (temporary teeth, flipper, nothing).
- Soft-food rules and how long they last.
- When the final prosthesis is seated, and whether that needs a second trip.
- Who adjusts the temp if it cracks while you are back in Arizona.

Extra-cost triggers to find before you sit down
Ask which of these are included, possible extras, or not offered — on paper:
- Extractions teeth (count and which ones).
- Bone graft or sinus work — overview: bone grafts for implants.
- CBCT / 3D imaging vs 2D films only.
- Sedation or IV anesthesia.
- Soft-tissue grafts, temporary dentures, night guards.
- Pharmacy meds after surgery (separate from the dental lines).
- Remakes if shade or fit fails.
You are not asking them to invent a worst-case novel. You are asking which common forks already have a line — and which forks mean “we stop and re-quote.”
Warranty terms in writing — not a hallway promise
Before you pay a major deposit, the plan (or an attached warranty sheet) should answer:
- Years of coverage for implant body vs prosthesis vs workmanship.
- What counts as failure (fracture, mobility, lab defect) vs patient factors they exclude.
- Whether you must return to the same clinic for warranty work.

Print this and mark each box on the clinic’s PDF:
- Patient name matches your ID.

- Every procedure is a line — not only a package title.
- Brands / materials named.
- Temp vs final and trip count stated.
- Imaging and grafts: included or extra, spelled out.
- Warranty sheet attached or linked in writing.
- Exclusions listed (hotel, flights, meds, remakes rules).
- Payment schedule and what is due before anesthesia.
- Signature / date fields for you and the clinic.
If three or more boxes fail, you do not have a plan yet — you have a pitch.
