- Digital X-rays (panoramic / bitewing / periapical) — PDF or JPEG preferred
- Treatment history (fillings, crowns, root canals, extractions)
- Periodontal charting if you have it
- Meds list and relevant medical notes
- If you already have implants: brand and model on paper

Email, secure upload, or USB the day you walk Andrade — ask this clinic which format they want. Bring a backup on your phone.
If you have no records
That is common. Ask: CBCT / X-rays on site today? before surgery day. A flat panoramic is not enough for implants. Same rule as questions before the chair and how to choose.
